Have you ever stopped to acknowledge your saliva glands? I know I hadn’t. Not until they stopped working for my husband. The oncologist warned him, they would be destroyed on his right side and possibly all together. When it comes to radiation for throat cancer, saliva glands are in the crossfire.
As I watch my husband navigate a world without this simple fluid, I’m learning gratitude for those tiny, tireless saliva factories tucked throughout my mouth and my throat.
Right now, as you’re reading this, hundreds of glands are working throughout your mouth. You have three pairs of major ones—the parotids near your ears, the submandibulars beneath your jaw, and the sublinguals under your tongue. But you also have 600 to 1,000 tiny minor glands scattered across your lips, cheeks, tongue, and palate. Together, they’re producing somewhere between half a liter to a liter and a half of saliva every single day.
What are they making? Mostly water, yes—99 percent. But that remaining one percent is performing quiet miracles. Enzymes are beginning your digestion before you even swallow. Antimicrobial compounds are fighting bacteria. Minerals are strengthening your tooth enamel. Proteins are lubricating everything, making it possible to speak clearly and swallow effortlessly.
To add to my husband’s misery, saliva is the reason you can taste anything at all. Your taste buds need food molecules to be dissolved in liquid to reach their receptors. Saliva is that solvent, breaking down the chemical compounds in your food so your brain can register flavor. Without it, taste receptors sit there, unable to do their job. It’s like trying to taste something through dry paper.
You’re not thinking about any of this. You’re just living, tasting, enjoying. That’s the gift.
For my husband, eating a piece of bread has become an ordeal. Without saliva, the bread doesn’t soften. It doesn’t form into that smooth, cohesive ball that slides easily down your throat. Instead, it turns into a dry, crumbly mass that sticks to his tongue, his cheeks, the roof of his mouth—almost adhesive in texture. He takes a bite, then immediately reaches for water. Another bite, another sip. The food feels rough going down, like he’s pushing. Even foods that should be moist become challenging without that natural lubrication. Every meal requires constant sips of water, and even then, there’s a sensation of thickness, of food coating his mouth that won’t clear.
Eating used to be one of life’s reliable pleasures. Now it’s work, and the food that’s already difficult to chew and swallow also tastes flat, muted, almost like nothing. There’s no flavor reward for all that effort. No pleasure in eating, only the mechanics of getting nutrition. Water helps a little, providing some liquid to dissolve food compounds, but it’s not the same as natural saliva. The simple joy of tasting his favorite foods is gone.
Recovery is possible. Over the next six months to two years, some salivary function might return. Maybe 25 percent of what he had before. Maybe 50 percent. But complete recovery is unlikely. The cells that produce saliva are delicate, and high-dose radiation damages them permanently.
So he has to learn to adapt. Artificial saliva products. Prescription medications that might stimulate whatever function remains. Meticulous attention to dental care, because without saliva’s protective coating, tooth decay becomes a serious risk. He’s discovering which foods work—soups, stews, anything with sauce or gravy. He’ll probably have to carry a water bottle everywhere, for the rest of his life. This is Wayne’s new normal.
I’ve never thought of saliva glands, not until now. They’re performing hundreds of small, essential tasks that make your life not just possible but pleasant. Thanks to saliva glands, you can eat a sandwich without thinking about it, taste the sweetness of a ripe peach or the richness of chocolate, and speak without your tongue sticking to the roof of your mouth. You can swallow your lunch, kiss someone you love—all without effort, all without awareness.
